Baylor has created a fact sheet with more information. Click here to download as a two-page PDF.

You need to make an appointment for the mammogram. Call 214-820-3040 to schedule an appointment. You will need to bring your insurance card and photo ID to the appointment.

An annual mammogram is covered by most insurance plans, with some fees billed at a later date. Mammograms under age 35 require a physician order.
